Paras Health Panchkula's Advanced Multidisciplinary Approach Helps Save 70-Year-Old Patient with High-Risk Renal Cancer

Panchkula 5 June ( Ranjeet Singh Dhaliwal ) : Paras Health Panchkula successfully treated a 70-year-old woman diagnosed with advanced kidney cancer that had extended into the Inferior Vena Cava (IVC), one of the body's largest veins. Cases of this nature are considered among the most complex in urologic oncology and require extensive surgical expertise and multidisciplinary coordination.

The patient had been experiencing weakness, loss of appetite and abdominal discomfort. Further investigations revealed a large tumour in the right kidney. Imaging studies showed that the cancer had extended into the IVC, the major vein responsible for carrying blood back to the heart. Initial scans suggested that the tumour thrombus had reached a high level near the liver, making the surgery particularly challenging.

The procedure was led by Dr Dushiant Sharma, Associate Director – Urology, Uro-Oncology & Robotic Surgery, along with Dr Karan Midha, Senior Consultant – GI, HPB & Liver Transplant Surgery, supported by a multidisciplinary team of anaesthesiologists, critical care specialists and surgical experts. During the surgery, the team used Transesophageal Echocardiography (TEE) to continuously monitor the tumour thrombus within the vein and minimise the risk of complications. To safely access the affected vessel, Dr Midha performed complete liver mobilisation, a highly specialised technique used in complex surgeries involving major blood vessels.

During the procedure, a direct Doppler ultrasound assessment revealed that the tumour thrombus was located lower than initially indicated on imaging scans. This crucial finding enabled the surgical team to modify the operative plan and avoid a more extensive procedure that could have required cardiac bypass support.

The team subsequently performed a successful right radical nephrectomy along with complete removal of the tumour thrombus from the IVC, restoring normal blood flow through the vein. The patient remained stable throughout the surgery and was shifted for close postoperative monitoring. Commenting on the case, Dr Dushiant Sharma said, "Managing kidney cancers that extend into major veins is among the most complex challenges in urologic oncology. In this case, advanced intraoperative imaging and close multidisciplinary collaboration helped us safely remove both the kidney tumour and the venous thrombus while minimising surgical risk."
